Summary
Did you catch the amazing trial results from 2016 and since that found intratracheal budesonide mixed with surfactant to deliver it to the distal airways dramatically reduced BPD in very low birth weight infants?
We did, and we thought maybe, just maybe, we had found the BPD SILVER BULLET!
That led to the international PLUSS trial conducted in Australia, New Zealand, Canada and Singapore, and published in 2024, which (to our dismay) found no significant improvement in survival free of BPD with intratracheal budesonide mixed with surfactant in extremely preterm infants.
At the same time, the similar BiB trial from the USA was underway, and the results were published very recently.
This gives us a great opportunity to review and discuss the status of early intratracheal budesonide and whether it still has a place in the care of extremely preterm infants. And if not, what next??
